CHWs in Action: Real Community Impact

“I was able to use findhelp.org and came across help for a patient who has pancreatic cancer, they can apply for a grant to help pay up to $2000 in bills and sign up for healthy meals for 2 months to be delivered to them. It made me feel great to help the patient but also help his girlfriend who was not adherent to her medications due to money and had lost her first husband to cancer too.”

"I spoke with a patient who was not adherent with her medications. She was getting evicted from her home and her medications were the last thing on her mind. She was able to contact a resource I had found during my training, and they were able to assist her in her rent for the month so she could avoid being evicted."

“We had a patient this summer that had to move in with his mother when we had a heat wave. He had no AC at his home. We were able to arrange delivery to his mothers home for his medications. I was also able to refer him to local county agency for assistance with emergency cooling program that provides window air conditioning. He and his mother were very appreciative of all the additional assistance we provided beyond just his medication needs.”

“We have a patient that just recently lost his wife and calls every 3 weeks when his meds are due to be filled in our pill pack program. He likes to talk to whoever answers the phone but especially to me. He says that he is lonely since she passed. I now have resources available to me that I gave him some resources to go for socialization. When he called this week he said he started to go to the Senior Center and is enjoying his time down there.”
